What are the steps to access benefits for MVA assessment and treatment?
1

Please
contact
your
Auto

Health
Benefits
provider
to
confirm
that
you
are
covered
and
inform
them
that
you
would
like
to
start
psychological treatment for the condition or symptoms related to MVA at ARCH Psychological Services.
2
.
MVA
insurer
might
ask
for
a
doctor's
note
where
it
is
stated
that
psychological
treatment
is
needed.
Please
also
email
the
note
to
us
at

.
We will book you for an assessment with the therapist and afterwards we will send the assessment report to your MVA insurer.

.
Once
we
get
the
approval
for
treatment
from
your

book
treatment

Progress
of
the
therapy
will
be
monitored and further treatments may be requested, if needed.
Please note the steps involved regarding processing fees:
Your

services,
before
billing

directly.
If
your

not
allow
us
direct

we
will
ask
you
to
pay
for
psychology
services
just
before
each
appointment
and
we
will
issue a receipt that you can submit to your extended health insurer and ask for reimbursement.
Once
you

insurer,
they
will
issue
you
a
letter
stating
there
is
no
more
coverage
available.
Please
send us this form, which we will submit to the auto insurer on your behalf.
